1. Call Holistic Evaluations at 206-303-0246

2. First time patients must be seen in-person, in-office with Doctor (WA law)

3. Bring a valid Washington State I.D. to your appointment

4. Bring myChart account Health Summary page www.mychart.org

5. Check in at front desk 15 minutes prior to appointment to fill out intake forms

6. Meet with the Doctor to recieve your authorization

7. Renewals may be scheduled as a TeleHealth appointment
