Welcome to Holistic Evaluations! Your initial visit will consist of a consultation with the doctor and 1 legal authorization effective for 1 year.

If Medical Records are taking longer to get ahold of than what is ideal, we can still offer the option to start with a 6-month authorization while we wait. Medical Records will need to be on file at the time of your next renewal. 

A second authentic authorization can be kept in a car, held in an employee folder at work, or filed for court reasons. Always keep one filed away somewhere secure!

Welcome back! Renewals are written for 1 year. 

A designated provider is someone to be added to your authorization who can go to a medically endorsed cannabis shop and pick up product on your behalf in the case that you are unable to go yourself. 

If your authorization has been lost or stolen, or you  need a new one due to a change of address or name change. Replacements for any reason will result in a replacement fee. 

Qualifying patients can also receive documents needed to obtain their disability parking placard, service only available at time of Medical Cannabis Authorization appointment. 

Patients who are facing legality restrictions may need to provide an added Legal Letter to the courts. This letter is an in-depth document that offers support to your Medical Cannabis Authorization as to why you use Medical Cannabis for your ailment/conditions. It is signed off by the doctor and and comes in a packet along with an additional Medical Cannabis Authorization as well as your printed Medical Records. This packet is $190.00 if ordered separately from your scheduled appointment.
