Welcome to Holistic Evaluations! Your initial visit will consist of a consultation with our marijuana doctor and 1 legal Washington State medical card authorization.
Additional MMJ Card

A second authentic authorization can be kept in a car, held in an employee folder at work, or filed for court reasons. Always keep one filed away somewhere secure! When you need it you will have it!
MMJ Card Renewal

Welcome back! Renewals are written for one year. We appreciate your continued business and being a part in the Holistic Evaluations extended family and success!
Designated Provider
$100.00
A designated provider is someone to be added to your authorization who can go to a medically endorsed cannabis shop and pick up the product on your behalf if you are unable to go yourself.
Replacement MMJ Card
$100.00
If your authorization has been lost or stolen, you need a new one due to a change of address or name change. Sometimes these things happen. Replacements for any reason will result in a replacement fee.
No Show Fee
