Patients needing a Covid Letter for work or travel purposes can schedule an appointment with our provider for a consultation to receive an approval letter (if qualified).
Disability Placard

Qualifying patients will receive the completed legal documents needed to acquire a disability placard for their car and/or transportation needs.
Note: We do not bill any insurance plans however, we will provide you with documentation to submit to your insurance company for potential reimbursement. Cash and all major credit cards are accepted.
