To Apply for Your Medical Cannabis Authorization, Please Submit the Following:
- WA state Photo ID
- Intake Forms (see documents below)
- Medical records that support the qualifying conditions (required for patients under 21).
Please complete all forms and email them to: holisticevaluations@gmail.com
Once your documents have been received, we will review them and contact you to schedule the appointment.
